What is the CLIF-SOFA score?
The CLIF-SOFA (Chronic Liver Failure - Sequential Organ Failure Assessment) score is a structured way to quantify organ dysfunction in patients with advanced liver disease, especially in acute decompensation and acute-on-chronic liver failure (ACLF) settings. It adapts concepts from the classic SOFA score to better reflect cirrhosis-specific physiology.
Each major organ domain receives points from 0 to 4. Higher points indicate worse dysfunction, and the summed total helps clinicians track severity over time.
Domains included in this calculator
1) Liver function
Estimated from total bilirubin. Rising bilirubin typically reflects worsening hepatic excretory dysfunction.
2) Kidney function
Estimated from serum creatinine and dialysis status. Renal replacement therapy is treated as severe renal dysfunction.
3) Brain function
Estimated from hepatic encephalopathy grade (West Haven scale), ranging from no encephalopathy to coma.
4) Coagulation
Estimated from INR. In cirrhosis, coagulopathy is complex, but INR remains a commonly used severity marker.
5) Circulation
Estimated using MAP and vasopressor/inotrope requirements. Increasing hemodynamic support usually signals worse circulatory failure.
6) Respiration
Estimated from PaO2/FiO2 and whether ventilatory support is required.
How to interpret the total score
The total score ranges from 0 to 24. While exact interpretation varies by institution and study population, a practical framework is:
- 0-5: Mild overall dysfunction
- 6-9: Moderate dysfunction; close monitoring needed
- 10-14: Significant multi-organ involvement
- 15+: Very high severity; urgent intensive management context
Also pay attention to the number of domains with scores 3-4, as this often corresponds to organ failure burden.
Why serial trends matter more than one number
A single CLIF-SOFA value gives a snapshot. The trajectory over 24-72 hours can be more informative:
- Improving score may indicate response to treatment.
- Stable high score may suggest persistent critical illness.
- Rising score can signal rapid deterioration.
For that reason, many teams reassess repeatedly rather than relying on one isolated measurement.
