DXM Dosage Calculator (Therapeutic OTC Guidance)
This calculator is for standard cough-suppressant use of dextromethorphan (DXM) only. It is not for recreational dosing. Always follow your product label and ask a clinician if unsure.
How to use this DXM dosage calculator
Dextromethorphan (DXM) is a common cough suppressant found in many over-the-counter cold medicines. The safest way to use DXM is to stick to labeled therapeutic dosing, avoid duplicate ingredients, and keep total daily intake below recommended maximums.
This tool estimates standard OTC guidance by age and formulation, then converts the dose into mL (for liquid products) or tablets/capsules (for solid products) if you enter product strength.

Important safety points before taking DXM
1) Check combination products
Many cough-and-cold medicines combine DXM with other active ingredients. Accidental overdose is common when people combine multiple products.
- Acetaminophen (APAP): too much can severely injure the liver.
- Diphenhydramine / antihistamines: can increase sedation and confusion.
- Pseudoephedrine / phenylephrine: may raise heart rate and blood pressure.
- Guaifenesin: generally compatible, but still count every ingredient.
2) Watch for medication interactions
DXM can interact with serotonergic medicines and raise the risk of serotonin toxicity. Use caution or avoid DXM unless approved by your prescriber if you take:
- SSRIs, SNRIs, MAOIs, tricyclic antidepressants
- Linezolid, methylene blue, tramadol, certain migraine medications
- Other substances that affect serotonin
3) Avoid alcohol and sedating substances
Combining DXM with alcohol or sedatives increases impairment risk. Do not drive or operate machinery if drowsy or cognitively slowed.
Signs of DXM side effects or overdose
Get urgent medical help if severe symptoms occur. Red flags include:
- Severe confusion, agitation, hallucinations, or unresponsiveness
- Rapid heartbeat, high fever, muscle rigidity, tremor
- Trouble breathing, persistent vomiting, seizures

Frequently asked questions
Is DXM dosage by weight or by age?
OTC labels usually dose by age ranges. Weight can add context, especially in pediatrics, but the package label and clinician guidance should lead.
Can I take two different cough medicines together?
Only if you verify they do not duplicate active ingredients. Two products may both contain DXM, acetaminophen, or antihistamines.
What if I missed a dose?
Take the next dose only when due. Do not double-dose to catch up.
