What is the FSFI?

The Female Sexual Function Index (FSFI) is a standardized questionnaire used in research and clinical settings to assess key dimensions of female sexual function over the previous four weeks. It includes 19 items grouped into six domains: desire, arousal, lubrication, orgasm, satisfaction, and pain.

Because it is structured and validated, FSFI scoring can help track change over time and support conversations with a qualified healthcare professional.

How FSFI scoring works

Each domain is calculated by summing specific item responses and applying a domain factor. Each domain has a maximum score of 6, and total FSFI score ranges from 2 to 36.

Domain Questions Factor Formula
Desire Q1–Q2 0.6 (Q1 + Q2) × 0.6
Arousal Q3–Q6 0.3 (Q3 + Q4 + Q5 + Q6) × 0.3
Lubrication Q7–Q10 0.3 (Q7 + Q8 + Q9 + Q10) × 0.3
Orgasm Q11–Q13 0.4 (Q11 + Q12 + Q13) × 0.4
Satisfaction Q14–Q16 0.4 (Q14 + Q15 + Q16) × 0.4
Pain Q17–Q19 0.4 (Q17 + Q18 + Q19) × 0.4

How to interpret your total score

Common screening threshold

A frequently cited cutoff is 26.55. Scores below this threshold may indicate possible sexual dysfunction and suggest the value of further evaluation.

  • FSFI ≥ 26.55: Generally above common screening cutoff.
  • FSFI < 26.55: May indicate clinically meaningful concerns worth discussing with a clinician.

Important: a cutoff is not a diagnosis. Interpretation should consider context, relationship factors, stress, medications, hormones, pain conditions, and mental health.

Best practices when using an FSFI calculator

  • Answer based on the same recall period (typically the prior 4 weeks).
  • Use the same scoring method each time for consistency.
  • Track domain scores, not just total score, to identify specific problem areas.
  • Use results as a starting point for medical discussion, not self-diagnosis.

When to seek medical support

Consider speaking with a qualified healthcare professional if symptoms are persistent, distressing, or affecting your quality of life or relationship. Seek care especially if there is ongoing pain, bleeding, major change in function, trauma history concerns, or mood symptoms.

Frequently asked questions

Is this calculator diagnostic?

No. It is a scoring tool for education and screening support only.

Can I use FSFI to monitor progress?

Yes. Repeated scores can help track patterns over time, especially by domain.

What if I am unsure about a response?

Choose the option that best reflects your experience in the recent recall window and make a note to discuss uncertainty with your clinician.
