International Prostate Symptom Score (IPSS) Calculator
Answer all 7 symptom questions based on the last month. Each question is scored from 0 to 5. The total score ranges from 0 to 35.
What is the IPSS?
The International Prostate Symptom Score (IPSS) is a validated screening tool used to measure lower urinary tract symptoms, especially in men with possible benign prostatic hyperplasia (BPH). It helps translate symptoms into a numeric score so that changes can be tracked over time.
Clinicians often use IPSS during initial evaluation and follow-up visits. Patients can also use it at home to understand whether symptoms are stable, improving, or worsening.
How scoring works
The seven symptom questions are each scored from 0 to 5, for a total possible score of 35.
| Total IPSS | Symptom Severity | Typical Interpretation |
|---|---|---|
| 0–7 | Mild | Symptoms are present but usually less disruptive. |
| 8–19 | Moderate | Symptoms may affect daily life and sleep. |
| 20–35 | Severe | Symptoms are significant and often need medical evaluation. |
Quality of life question
The separate quality of life (QoL) item is scored from 0 to 6. It does not change the 0–35 symptom total, but it helps explain how bothersome symptoms feel to the patient.
Why this calculator can be useful
- Provides a structured way to describe urinary symptoms.
- Improves communication between patient and clinician.
- Helps monitor symptom trend before and after treatment.
- Supports shared decision-making around lifestyle changes, medication, or further testing.
When to seek medical care promptly
Even with a low or moderate score, you should seek prompt care if you have warning signs such as:
- Blood in the urine
- Painful urination with fever/chills
- Inability to urinate (urinary retention)
- Severe lower abdominal pain
- Sudden major worsening of symptoms
Tips for getting better symptom control
1) Lifestyle adjustments
Reducing evening fluid intake, limiting caffeine/alcohol, and planning timed voiding can decrease urgency and nighttime urination in some people.
2) Medication review
Some medications can worsen urinary symptoms. Reviewing your medication list with your clinician may identify reversible factors.
3) Follow-up and re-scoring
Repeat your IPSS periodically (for example every 4–12 weeks during symptom changes or treatment initiation) to evaluate progress objectively.
