What Is the LRINEC Score?

LRINEC stands for Laboratory Risk Indicator for Necrotizing Fasciitis. It combines six routine laboratory values into a point-based score (0 to 13) to help stratify concern for necrotizing soft tissue infection.

Necrotizing fasciitis is a rapidly progressive, life-threatening condition. Early recognition is critical. LRINEC was proposed to provide a standardized way to organize lab findings when clinical suspicion exists.

Variables and Point Assignment

  • CRP (mg/L): < 150 = 0 points, ≥ 150 = 4 points
  • WBC (×10³/µL): < 15 = 0, 15–25 = 1, > 25 = 2
  • Hemoglobin (g/dL): > 13.5 = 0, 11–13.5 = 1, < 11 = 2
  • Sodium (mmol/L): ≥ 135 = 0, < 135 = 2
  • Creatinine (mg/dL): ≤ 1.6 = 0, > 1.6 = 2
  • Glucose (mg/dL): ≤ 180 = 0, > 180 = 1

How to Interpret LRINEC

Low Risk (0–5)

A lower score may suggest lower probability, but it does not safely exclude necrotizing infection in a concerning clinical scenario.

Intermediate Risk (6–7)

This range should raise concern, especially if pain is out of proportion, systemic toxicity is present, or skin findings are rapidly evolving.

High Risk (8–13)

A high score is strongly concerning. Immediate escalation, broad-spectrum antimicrobial treatment per protocol, and urgent surgical consultation are often warranted.
